III. Documents required for sign-up

1. 營業執照、稅務登記證（三證合一的可僅提供合并后的證書）、組織機構代碼證（三證合一的可僅提供合并后的證書）；

1. Business license, tax registration certificate (in case of the three certificates in one,provide the combined certificate only), organization code certificate (in case of the three certificates in one, provide the combined certificate only);

2. 材料品牌制造商出具的授權書；

2. Power of Attorney issued by material brand manufacturer;

3. 法定代表人證明書或法定代表人授權委托書、被授權人身份證復印件。

3. Certificate of legal representative or authorized representative’s power of attorney, authorized person’s copy of ID card.

4. 業績證明材料（提供合同及用戶使用證明復印件，包括合同完成情況、畫面使用單位、畫面使用單位聯系方式等）；

4. Performance proof material (provide a copy of the contract and use proof, including the completion of contract, user unit, user unit’ contact method, etc.);

5. 固定服務場所的租賃合同或產權證。

1）以上提交的資料均須提供原件和復印件，原件審核后退回，復印件須加蓋公章。如有缺漏，招標代理機構將拒絕接受其報名。

四、招標文件發售時間和地點

IV. Time and place for sale of the Bidding Documents

凡有意參加投標者請于2017年3月9日至2017年3月13日每日上午 8:30 時至 11:30 時，下午 13:00 時至 16:00 時，委派授權代表持上述資格證明文件，至上海百通項目管理咨詢有限公司（上海市浦明路1229弄底樓西大廳）報名，經驗證后領取招標文件，本次招標文件收取工本費人民幣500元/套，售后不退。

Tenderers who intends to participate in the bid shall appoint the authorized representative to carry the above qualification certificates and sign up in Shanghai Baitong Project Management Consulting Co., Ltd. (West Hall, Ground Floor, Lane 1229, Puming Road, Shanghai) at 8:30~11:30, 13:00~16:00 between 9 March, 2017 and 13 March, 2017, after verification obtain the Bidding Documents. The fee for the Bidding Documents is 500 yuan/set. No return, no refund after purchase

五、投標截止及開標時間：2017年3月31日上午09：00時（逾期送達或不符合規定的投標文件恕不接受。）
